Personalized Weight Management Plan

Weight management is a personal journey that begins with a tailored program designed to help patients develop the behaviors supporting a healthy lifestyle.

Starting with a consultation appointment, an Obesity Case Manager will work with the patient to develop a personalized lifestyle, education, and behavior modification plan. The patient works with a multidisciplinary care team for four to six months to implement the plan, during which a surgical pathway may be developed.

Bariatric Surgery Program Eligibility

  • Age: 18- 60 years of age
  • Body Mass Index (BMI): Above 35
  • JHAH registered patient (JHAH Facilities or Medical Designated Facilities, Eastern Province)
