The gastroenterology team led by Doctor Diamond Joy performed the first pH of manometry service and the first case of adult esophageal manometry at JHAH.
Dr. Joy completed an NHS contract at University Hospitals in Oxford and North Durham University Hospital, England, where he learned about esophageal pH, impedance, and esophageal and anorectal manometry techniques in 2021.
As part of JHAH’s commitment to knowledge transfer, Dr. Joy proctored at the King Fahad Hospital in Dammam and shared his experiences with esophageal pH and esophageal and anorectal manometry recording.
Esophageal physiology studies are usually undertaken to assess the function of the esophagus. Functional esophageal disorders represent processes usually accompanied by typical esophageal symptoms (heartburn, chest pain, dysphagia, globus).
The most common esophageal disorder occurs when the lower esophageal sphincter doesn’t close properly. As a result, stomach acid and contents flow backward into the esophagus, causing acid reflux (Gastroesophageal reflux disease- GERD). In addition, other conditions cause functional disturbances, e.g., Achalasia. Some patients are considered for surgical or endoscopic intervention.
This test looks at how well the food pipe (esophagus) works as a swallowing tube. A tube passes down the food pipe. This tube has several pressure sensors spread at intervals along its length. The test observes the pressure within each part of the esophagus as the swallow progresses. As patients swallow, the upper and lower esophageal sphincters should relax at appropriate moments. This test assesses the swallow’s efficacy and diagnoses esophageal motility disorders such as Achalasia.

The second part of the test involves an assessment of the extent to which reflux of acid occurs. The tube used has several sensors which detect acid. This tiny tube is connected to a small box to which the recordings are downloaded and analyzed. The tube usually remains in place for 24 hours.
